Involve patients and their loved ones as part of the health care team, collaborating with medical professionals in making clinical decisions about their care
Put into the hands of patients responsible for important aspects of personal care and monitoring, along with the tools and support they need
Ensure that transitions between providers, departments and health care settings are respectful, coordinated and effective
